ADD/ADHD testing can be expensive. The good news is that your insurance will often cover some, if not all of the costs. Even if Kalamazoo ADHD is not directly paneled with your insurance company, we can provide you with information to be reimbursed following your out of pocket expense.

Please know that a $250 deposit is required for testing and is due prior to the first testing appointment (not the initial consultation).
This is not a retainer, down payment, prepayment, or gift to Dr. Barnes. Often times the testing process happens quicker than an insurance company can respond with their coverage. As a result, we ask for a $250 deposit to apply towards any uncovered expenses. At the end of the testing process, when your insurance company is able to respond to our charges, any leftover funds will be immediately refunded to you.
If this is an out of pocket expense, 50% of the agreed upon costs are due prior to the beginning of ADD/ADHD testing.

Dr. Barnes asks that you contact the customer service number on the back of your insurance card and ask whether the CPT code 96136 and 96137 is covered. You will also want to ask whether authorization is needed as well. This is an important question because if preauthorization is needed, yet not attained, this testing could become an out of pocket expense. If you have any questions, please email Dr. Barnes and we will send over a document that outlines this process. You will receive this document when we do our interview as well.
